What is Ketamine?

Ketamine is a legal, safe and effective medicine used to treat a variety of mental health conditions, including depression, anxiety and PTSD. Ketamine has rapidly- acting antidepressant and mood-enhancing effects, which can begin to take effect within 1-2 hrs. after treatment.

It works by blocking the brain’s NMDA receptors as well as by stimulating AMPA receptors, which are thought to help form new synaptic connections and boost neural circuits that regulate stress and mood.

Ketamine has also been shown to enhance overall neuroplasticity for lasting symptom improvement. Ketamine can be administered in a variety of ways, including IV infusion, intramuscular injection, via nasal spray and using sublingual lozenges. In my work with Journey Clinical we only use the sublingual lozenge form.

Costs

How Does Ketamine Feel?

The effects of ketamine, which most patients find pleasant, last for approximately 45 minutes. These effects can make you feel “far from” your body, and facilitate shifts in perception that can often feel expansive in nature. Your motor and verbal abilities will be reduced, so you’ll be lying down in a comfortable position during the experience.

Once these effects subside, we’ll spend the remainder of our appointment giving you space to process and discuss your experience. While it may feel hard to articulate what happens during the experience, patients feel like the insights gained are clear.

It is estimated that these treatments pay for themselves over 3 years time, comparing them to taking daily prescription medication, doctors visits, lost time from work, family time and ultimately, these medicines save countless lives!

Currently in Texas, Ketamine is the only medicine legally available with MDMA and Psilocybin in a therapeutic setting coming within two years.  Ketamine was approved decades ago as an anesthetic by the FDA.  It is a “dissociative anesthetic” used frequently in children.  It is a Schedule III controlled substance.  It was a common battlefield anesthetic used in the Vietnam war.

There is an unmet need in treatment for depression in our current medical model.  Up to ⅓ of all patients will fail to respond to multiple attempts at treatment, meaning trying different antidepressants.

After failing 2 antidepressants, the likelihood of a third antidepressant medication working drops to 16%.  This leaves a large part of the population with disabling depression.  Ketamine is thought to work because it works on a different system in the brain than common antidepressants.

Instead of targeting the serotonergic system, ketamine works on the glutaminergic system within the brain, decreasing inflammation throughout the brain and body at the same time, creating neurogenesis and neuroplasticity.

Ketamine can be administered most commonly through an IV at a clinic, by intramuscular injection, which is less commonly used, or by oral lozenge.  Each of these methods have distinct costs separate from the cost of the psychotherapist on hand.  These medications are currently not covered by insurance although there is a ground swell of protest to have them covered.

Most clinics will run packages that if you pay for the first series, which is a series of 6 sessions over 2 weeks, the price is lowered to $400 per session.  Some clinics insist you pay up front, but some clinics allow you to pay per session.  I use a clinic in McAllen, TX that will let you pay per session.

I recommend you have a psychotherapist with you if your budget allows, because it can be an intimidating experience and the support you receive from someone who knows you and is trained in being supportive can make a big difference.  Psychotherapists will usually charge a minimum of two hours time and the fees can be from $150-$300 per hour.

Before going to a dosing session at a clinic, the clinical protocol is for you to see a psychotherapist for one to two sessions to go over preparation for the experience.  Then the first dosing session is performed and the following day, you will have an integration session with the psychotherapist.  Integration is as important as the dosing session.  You must integrate the information that came up in the session and begin to apply it to your current life.

Even though, it may seem expensive

there is a concerted effort to bring these healing medicines to more populations through creative funding and payment plans.  Most people want to be free from the struggle, the pain, the depression, the anxiety no matter what the cost.  When reviewed with the current cost of feeling all the pain, the medication taken to date, the doctors appointments, the time, the energy, the effects on the family… and still feeling lost, this is very achievable.
